This is an 18-year-old male who presented with shooting pain down his leg. Given the symptoms, the patient underwent an MRI of the lumbar spine to look for nerve root compression which was negative. Radiographs were subseqently obtained.
Frontal radiograph of the hip demonstrates abnormal cortical thickening or hyperostosis of the femur as indicated by the red arrows. This is better appreciated on radiograph of the femur more distally.
The imaging appearance of this disease has been described as having a "dripping candle wax" appearance. The imaging findings are consistent with a skeletal dysplasia known as melorheostosis.
Patients with this disorder often present with severe pain, often involving the entirety of the affected extremity. Pain often occurs along a dermatomal distribution, and the patients' symptoms are often intially confused as being secondary to an underlying neuropathy.
患者男,18岁,小腿下段刺痛/放射痛。结合病史行腰椎MRI检查显示神经根无受压。遂进一步检查。髋部正位片可见股骨远端骨皮质异常增厚或骨质增生(红箭头所指)。股骨远端平片可更好的显示这一征象。此种征象被称为“蜡泪征”,由于骨质发育不良导致,即蜡泪样骨病。此病患者常伴剧痛,疼痛波及整个病变骨全长,沿神经皮节分布。此症状在初期很容易被误认为是潜在的继发性神经病变导致。

1. cortical ['kɔrtɪkl] adj. 皮质的
Two types of bone found in the body----cortical and trabecular. Cortical bone is dense and compact. It forms the outer layer of the bone. Trabecular bone makes up the inner layer of the bone and has a spongy, honeycomb-like structure.
2. hyperostosis [ˌhaɪpərɒ'stoʊsɪs] n. 骨肥大,骨肥厚
Excessive or abnormal thickening or growth of bone tissue.
3.distally ['dɪstælaɪ] 远侧
Anatomically located far from a point of reference, such as an origin or a point of attachment; Situated farthest from the middle and front of the jaw, as a tooth or tooth surface.
4. dysplasia [dɪs'pleɪʒə] n. 发育异常,发育不良,(由发育异常形成的)异常结构
Abnormal development or growth of tissues, organs, or cells
5. melorheostosis [məlɔ:'hi:oʊstoʊsɪs] 蜡泪样骨病
An idiopathic defect of long bone growth, characterised by cortical thickening of long (tubular) bones which, when stripped of muscle, simulates a candle with wax melted down a side.
6. dermatomal [,də:-mə'təuml] adj. 皮区的;n. 皮节区范围
Dermatomal distribution involves an area of skin supplied with sensory innervation by a particular nerve root. Dermatomal distributions do not cross the midline of the body.
7. neuropathy [nʊ'rɒpəθɪ] n. 神经病变
A classic term for any disorder affecting any segment of the nervous system; In contemporary usage, a disease involving the cranial nerves or the peripheral or autonomic nervous system.
