Diagnosis: Aneurysmal bone cyst.
Discussion: Fluid-fluid levels on an MR image of an expansile lytic bone lesion were once thought to be pathognomonic for an aneurysmal bone cyst. However, multiple lesions have since been shown to contain fluid levels, including telangiectatic osteosarcomas and chondroblastomas. Fluid-fluid levels occur secondary to dependent stagnant blood products that layer within these dilated spaces. The blood products shorten the T2 relaxivity of the lesion and result in areas of relative T2 hypointensity.
